Complaints Form

Every patient has the right to make a complaint about the treatment or care they receive at Oldham Family Practice. We want to provide the best possible service, and your feedback helps us improve.

Step 1: Speak to Our Team (Local Resolution)

Most complaints can be resolved quickly by speaking directly with our clinic staff.

  • Please raise your concern with any member of staff, and they will assist you if possible.

  • Alternatively, you can ask to speak with our Complaints Manager. Please note: You may need to book an appointment to see them.

Step 2: Contact the Integrated Care Board (ICB)

If you do not wish to speak directly with our surgery staff, you can ask the NHS Greater Manchester Integrated Care Board to investigate your complaint on your behalf.

The GP Patient Survey is an independent survey run by Ipsos on behalf of NHS England. The survey is sent out to over two million people across the UK. The results show how people feel about their GP practice.
